Student Registration Form

- Sept. 4: T/TH and M-TH classes
- Sept. 5: M/W and MWF classes
MEET THE TEACHER VISITS: You and your child will have the opportunity to meet with their teacher one-on-one before school starts. Teachers will begin calling to schedule these visits in August after they receive class lists.
REGISTRATION FEE: An annual registration fee is due at the time of enrollment in order to secure your child’s spot. The 2018-19 registration fee is $125/family. It is discounted to $100/family during Early Registration February 5-March 16, 2018.
SEPTEMBER TUITION: September tuition is due as follows: by 5/11/18 for families enrolling between Feb.5-March 16, 2018; by 6/30/18 for families enrolling between March 17-June 30, 2018; at the time of registration for families enrolling on or after July 1, 2018 Multi student discount: Families enrolling more than one child receive a 10% discount off the additional child’s tuition. Payment in full discount: Families that pay the entire year’s balance by August 15, 2018 receive a 10% discount off the year’s tuition. To help us minimize payment processing fees, we ask that families paying the year in full pay by check.
REFUND POLICY: The registration fee is non-refundable. September tuition is non-refundable unless a class cancels due to low enrollment (<75% full) or in the event of a military relocation out of the area. If a class is cancelled due to low enrollment, you will be notified by July 13, 2018 and you will be given the opportunity to enroll in a different class or withdraw your child from the program.
IMMUNIZATION POLICY: All students must have proof of immunization. Calvary Preschool does NOT allow religious or personal exemptions. PLEASE SUBMIT IMMUNIZATIONS ON THE COLORADO IMMUNIZATION FORM. If your doctor provides you with a different form, you may be asked to transcribe information onto the Colorado form.
TOILET TRAINING: Children attending any classes EXCEPT Jr. Preschool (2 yrs. old) MUST be fully potty trained.
HEALTH APPRAISAL POLICY: State law requires all students to have a physical within the last 12 months prior to enrollment. The Health Appraisal From must be signed by both the physician and parent/guardian.
